Karlovy Vary spas and wellness centers range from physician-led medical spa programs to thermal pools, sauna worlds, massages, and short relaxation visits. The city’s spa tradition grew around its mineral springs, but a modern spa visit can mean very different things depending on whether you want treatment, a day of bathing and heat, or a hotel with its own wellness center.
The most important distinction is between medical spa care and wellness. Traditional Karlovy Vary treatment combines medical supervision with prescribed procedures and may use local thermal mineral water, natural carbon dioxide, peat, physiotherapy, diet, and a drinking cure. A wellness visit is more flexible: you might swim, use saunas and steam rooms, book a massage, or spend a few hours in a relaxation area without entering a medical program.
We spent a month in Karlovy Vary, which gave us time to see how deeply spa culture is woven into daily life in the city—from the historic bathhouses and medical spa hotels to public pools, sauna centers, and wellness facilities. We’ve combined that time in the city with current research on balneology, treatments, facilities, access, and spa etiquette to explain the choices travelers face and how the different types of spas compare.

Karlovy Vary Spas & Wellness Centers at a Glance
Karlovy Vary is unusual because medical spa treatment and leisure wellness still operate side by side. Choose the type of facility first, then compare individual treatments, pools, saunas, and access rules.
These are the main decisions before you book:
- For a traditional medical spa cure: Choose a medical spa hotel, spa house, or outpatient program with a spa physician and a treatment plan built around your health history.
- For a public day spa: Elisabeth Spa, Windsor Medical Complex, Saunia Thermal Resort, and Carlsbad Plaza’s outpatient/day-access services are the four facilities grouped under Karlovy Vary’s official public-baths directory, but they serve very different purposes.
- For public swimming plus a sauna center: Mattoni Arena (KV Arena) is a public pool-and-sauna complex, but it is not one of the four facilities in the city’s official public-baths category.
- For an actual mineral-water pool: Check the water source rather than assuming every warm pool is thermal. Saunia’s smaller outdoor pool is filled with 100% Karlovy Vary spring water; many other spa swimming pools use ordinary treated water.
- For saunas and steam rooms: Saunia and Carlsbad Plaza have the largest public heat-and-cold circuits, while many hotel spas combine Finnish saunas, steam rooms, infrared cabins, Kneipp paths, whirlpools, and smaller relaxation pools.
- For treatments without an overnight spa hotel stay: Elisabeth Spa sells individual treatments, while Carlsbad Plaza has an ambulatory medical program for non-hotel patients. Other facilities may accept outside clients for selected services, but day access should be confirmed directly.
- For a short relaxation visit: Choose wellness rather than a full medical cure. Massage, sauna, pool, private spa, beauty care, and relaxation packages do not require the same medical structure as physician-prescribed balneotherapy.
- For a classic Karlovy Vary cure: Expect medical assessment, a treatment schedule, and often a prescribed drinking cure. The city’s own spa guidance distinguishes this from a freer wellness stay and notes that traditional treatment is normally measured in weeks rather than a single afternoon.
- For clothing and gender rules: Do not assume one Karlovy Vary standard. Some pools require swimwear while adjoining saunas prohibit it; some sauna areas are mixed, some are separated for men and women, and some facilities offer private or women-only periods.
If you have only a few hours and want to bathe rather than be treated, Saunia is the clearest public thermal-pool choice. If you want the traditional medical side of Karlovy Vary, compare the physician-led public and hotel programs instead of choosing by the size of the sauna area.
Types of Spa Treatments in Karlovy Vary
Karlovy Vary treatment menus can look overwhelming because traditional balneotherapy sits beside modern physiotherapy, physical medicine, massage, and beauty treatments. The categories below explain what the main procedures are and whether they belong primarily to medical treatment, relaxation, or both.
Medical Spa Programs and Physician-Prescribed Treatment
A traditional Karlovy Vary medical spa stay starts with a medical assessment. The spa physician may prescribe a drinking cure, baths, physiotherapy, massage, carbon-dioxide treatments, heat treatments, inhalation, exercise, or other procedures. Longer programs often include follow-up examinations and dietary guidance.
This is different from picking treatments from a wellness menu. If a procedure is being used to treat a medical condition, the doctor decides whether it is suitable and how it fits with the rest of the program. Karlovy Vary’s own spa guidance separates physician-led classic treatment from flexible wellness stays.
Thermal Mineral Baths
Mineral-water bathing is one of the most recognizable forms of Karlovy Vary balneotherapy, but it usually happens in an individual treatment bath rather than a large communal swimming pool. Hotels and spa houses with certified access to the thermal water can pipe it into balneological departments for baths and other prescribed procedures.
Do not assume that a hotel with a swimming pool fills that pool with mineral water. Pools need their own water treatment and operating systems, and many are ordinary freshwater pools even when the same building uses Karlovy Vary mineral water in treatment tubs.
Hydrotherapy and Water-Based Rehabilitation
Hydrotherapy uses water for movement, pressure, temperature change, and massage. Depending on the facility, you may see whirlpool baths, underwater massage, Hydroxer baths, alternating water jets, Scottish showers, rehabilitation pools, water exercise, or Kneipp paths.
Some of these procedures appear on both medical and wellness menus. A whirlpool in a public pool complex is mainly for relaxation; underwater massage or a prescribed hydrotherapy session in a treatment department is a structured procedure.
Carbon-Dioxide Treatments
Karlovy Vary spa medicine also uses natural carbon dioxide. Facilities may offer carbonic mineral baths, dry carbon-dioxide baths, gas injections, or pneumopuncture. These belong on the medical side of the spa tradition and should not be confused with ordinary carbonated bath additives.
Because indications and contraindications matter, gas procedures are normally selected within a medical program rather than treated as a sightseeing experience.
Peat, Paraffin, Parafango, and Other Heat Treatments
Warm peat packs and baths use peloids—natural mud or peat materials—for localized heat. Other centers use paraffin or parafango packs. These treatments are commonly paired with musculoskeletal rehabilitation because they warm tissues before massage, exercise, or other therapy.
Heat treatments can also appear on relaxation menus, but medical programs adjust temperature, duration, and placement to the individual.
Physiotherapy and Rehabilitation
Physiotherapy is a major part of serious Karlovy Vary medical-spa care. Programs may include individual therapeutic exercise, group exercise, aqua exercise, manual therapy, posture and mobility work, lymphatic techniques, or Nordic walking.
This is one of the clearest differences between a medical spa and a leisure wellness center. A pool and sauna can be pleasant on their own; a medical resort adds assessment and a structured rehabilitation plan.
Electrotherapy, Light, Laser, Ultrasound, and Other Physical Therapies
Many larger medical centers combine balneotherapy with physical medicine. Treatment menus can include electrotherapy, magnetotherapy, ultrasound, laser or light therapy, shockwave therapy, and other equipment-based procedures.
These are not traditional mineral-water treatments simply because they take place in a spa town. They are modern therapies that medical spas combine with Karlovy Vary’s natural resources.
Inhalation, Oxygen, and Respiratory Treatments
Mineral-water inhalation appears in several Karlovy Vary balneological departments. Some resorts also add oxygen therapy, and Hotel Imperial has a hyperbaric chamber as part of its wider medical program.
These treatments are more clinical than a steam room. A steam bath is a warm, humid relaxation space; a prescribed inhalation procedure delivers a specific aerosol or therapy for a set period.
Massage and Manual Treatments
Massage is the bridge between medical treatment and wellness. Classic massage, reflex massage, underwater massage, lymphatic drainage, and manual techniques can be part of rehabilitation, while hot-stone, aroma, Thai, Ayurvedic, and similar massages are usually booked for relaxation.
The treatment name does not always tell you whether a physician is involved. Check whether you are booking a medical procedure, a wellness massage, or a package that combines both.
Relaxation, Beauty, and Specialty Wellness
Short spa visits often focus on aroma or herbal baths, private whirlpools, floating, salt rooms, cosmetics, body wraps, and non-medical massages. Karlovy Vary also has themed experiences such as beer baths.
These can be enjoyable parts of a trip, but they are not automatically balneotherapy. The defining feature of Karlovy Vary medical spa care is the controlled use of natural healing resources and prescribed procedures, not simply warm water, steam, or a treatment menu.
Balneology and the Karlovy Vary Spa Tradition
Balneology is the study and medical use of natural mineral waters, gases, peloids, climate, and related spa resources. In Karlovy Vary, it explains why the springs are treated as more than tourist fountains and why many hotels still have doctors, treatment departments, rehabilitation rooms, and dedicated mineral-water pipes.
What Balneology Means in Karlovy Vary
Karlovy Vary’s balneology combines local thermal mineral water with other natural resources and medical procedures. The water may be prescribed for drinking, used in individual baths and irrigations, or incorporated into hydrotherapy and inhalation. Natural carbon dioxide and peat are also used at some medical facilities.
Balneology does not mean that every spa procedure is “natural” or that every pool is mineral water. A modern medical spa may combine thermal baths with physiotherapy, laboratory tests, electrotherapy, laser treatment, exercise, and nutrition advice.
How a Traditional Medical Spa Stay Works
A classic stay begins with a physician rather than the sauna. The doctor reviews your medical history, may order tests, and builds a schedule of procedures. The plan can combine a prescribed drinking cure from Karlovy Vary’s springs with baths, rehabilitation, massage, heat, gas, inhalation, or physical therapies.
The city’s spa guidance recommends a substantially longer stay for traditional treatment than for a wellness weekend. Several resorts sell shorter introductory or relaxation programs, but a few nights with massages should not be described as equivalent to a multi-week medical cure.
Science and the Institute of Spa and Balneology
Karlovy Vary Region has a dedicated Institute of Spa and Balneology, a public research institution that studies spa medicine and the region’s natural healing resources. Its work includes scientific evaluation of balneological care rather than treating spa tradition as folklore alone.
That research role matters for travelers because health claims should come from qualified medical and scientific sources. A hotel brochure can tell you which treatments it sells; it cannot tell every traveler that a procedure is medically appropriate.
Why the Spa Town Was Built Around Treatment
Karlovy Vary’s grand hotels, colonnades, parks, promenades, and bathhouses developed around the daily rhythm of treatment. Patients drank at the springs, walked between procedures, bathed, rested, and returned for repeated treatment over a long stay.
This wider European spa-town system is why Karlovy Vary is one of the eleven Great Spa Towns of Europe on the UNESCO World Heritage List. The listing recognizes the urban form created by mineral springs, bathhouses, drinking halls, treatment facilities, parks, and promenades.
Imperial Spa: Historic Bathhouse, Not a Current Treatment Center
Imperial Spa (Císařské lázně, historically Spa I) is one of the city’s most important spa buildings, but it should not appear in a current treatment inventory. The historic bathhouse stopped serving its original balneological function in the late 20th century and has been restored as a cultural monument and visitor venue.

What You’ll Find Inside Karlovy Vary Spas & Wellness Centers
The word “spa” can describe a medical treatment department, a pool-and-sauna complex, or a small hotel wellness room. Compare the actual facilities before booking, especially if a thermal pool, specific sauna type, or cold-plunge circuit is important to you.
Swimming Pools
Large hotel spas may have a lap pool, rehabilitation pool, relaxation pool, or several pools with different purposes. Pool water is often ordinary treated freshwater. Elisabeth Spa’s 25-meter swimming pool is chlorinated freshwater, even though the same complex is a traditional Karlovy Vary treatment facility.
If you want to swim for exercise, pool size and lane layout matter more than the word “spa.” If you want mineral-water bathing, verify the water source separately.
Thermal Mineral-Water Pools
A true spring-water pool should say so clearly. At Saunia Thermal Resort, the smaller outdoor thermal pool is filled with 100% Karlovy Vary spring water and kept at about 38°C. Saunia’s larger pool is a separate swimming and relaxation pool at a lower temperature.
Elsewhere, mineral water may be reserved for individual baths rather than a communal pool. That is often more traditional to Karlovy Vary balneotherapy than swimming in thermal water.
Whirlpools, Hot Tubs, and Massage Pools
Whirlpools appear in many public and hotel spas. Some are stand-alone hot tubs; others are zones within a larger pool with jets, benches, countercurrent, or underwater massage elements. “Jacuzzi” is a brand name, so facility descriptions may instead say whirlpool or whirlpool bath.
A whirlpool is normally a relaxation facility unless it is part of a prescribed hydrotherapy treatment.
Finnish Saunas and Dry-Heat Rooms
A Finnish sauna uses high dry heat with relatively low humidity. It is the most common sauna type in Karlovy Vary hotel wellness centers and often forms the hottest room in a multi-stage sauna circuit.
Larger centers may add milder dry rooms such as bio, herbal, aroma, or salt saunas. Names vary by facility, so compare temperature and humidity rather than assuming every branded room follows one universal standard.
Steam Rooms, Turkish Baths, Caldariums, and Hammams
Steam rooms use lower temperatures and very high humidity. Facilities may label them steam bath, Turkish steam, caldarium, or hammam. Some hotels use “hammam” simply for the wet-heat room rather than a full traditional hammam bathing ritual.
If humid heat is important to you, check the room list rather than assuming a hotel with “sauna” also has steam.
Infrared Cabins and Tepidariums
Infrared cabins warm the body with radiant heat at a lower air temperature than a traditional Finnish sauna. Tepidariums are gentler warm rooms designed for longer, quieter rest between hotter or colder stages.
Carlsbad Plaza and several hotel spas combine multiple heat intensities, which is better for travelers who do not want every sauna session to be very hot.
Cold Plunge Pools, Ice Wells, Buckets, and Cooling Showers
Sauna circuits usually need a cooling stage. Depending on the facility, that may mean a cold plunge pool, ice well or fountain, bucket shower, cooling mist, or ordinary cold shower.
These facilities are part of the sauna experience, not a medical cold-therapy prescription. If you have cardiovascular or other health concerns, follow medical advice rather than copying another visitor’s heat-and-cold routine.
Kneipp Paths and Alternating Water
Kneipp facilities alternate warm and cold water around the feet and lower legs. They may be built as shallow walking basins, paired foot pools, or short treatment circuits.
Hotel Thermal, Anglický Dvůr, Richmond, and other Karlovy Vary wellness areas include Kneipp-style facilities alongside pools and saunas.
Salt Caves, Relaxation Rooms, Fitness, and Private Spa Areas
Salt rooms or caves, quiet relaxation rooms, loungers, fitness rooms, beauty salons, and private spa suites round out many wellness centers. These are not substitutes for balneotherapy, but they can matter more than the medical menu when your goal is a few hours of relaxation.
Private spa rooms are also worth considering if you want a whirlpool or sauna without sharing a large public complex.
Spa Etiquette, Clothing & Gender Rules in Karlovy Vary
Karlovy Vary does not have one dress code for every spa. The rule can change as you walk from a swimming pool into a sauna, steam room, treatment room, or private suite. Check the facility you plan to visit rather than assuming that a swimsuit is either always required or always forbidden.
Pool, Whirlpool, and Thermal-Pool Etiquette
Public swimming and bathing zones generally expect purpose-made swimwear, but each operator sets the exact rule. Elisabeth Spa’s current pool rules require clean swimwear and prohibit ordinary underwear, non-swimming shorts, burkinis, swimming hijabs, and full-body suits. Mattoni Arena publishes a similar rule. At Saunia Thermal Resort, visitors must wash and shower without a swimsuit before entering the pool, then use clothing suitable for bathing in the pool zone.
Shower before entering a pool or whirlpool, and do not carry a wet swimsuit rule from the pool into the sauna. In several Karlovy Vary facilities, the pool is a swimsuit zone while the sauna next door is textile-free.
Sauna and Steam-Room Clothing
Textile-free sauna use is common, but it is not universal enough to assume. Saunia Sauna World permits entry only without a swimsuit. Elisabeth Spa requires guests to remove swimwear in both the sauna and steam cabin, and Mattoni Arena requires visitors to remove swimwear before its sauna. Bristol Group likewise states that its Finnish sauna is entered only after a shower and without a swimsuit.
In a dry sauna, put a sheet or towel under your whole body, including your feet, when the operator requires it. Steam-room rules can differ because a wet sheet may not be used the same way. Follow the posted instructions for the exact room rather than applying the Finnish-sauna rule everywhere.
Mixed-Gender, Separate, Women-Only, and Private Areas
Gender arrangements vary just as much as clothing rules. Parkhotel Richmond has separate Finnish saunas for men and women, plus a shared herbal sauna and shared salt tepidarium. Elisabeth Spa separates its sauna and steam areas by sex but uses a shared post-sauna rest room. Mattoni Arena reserves its sauna for women on Wednesdays from 16:00 to 21:00. Hotel Thermal reserves one weekday-morning hour in its indoor wellness area solely for women.
Private wellness changes the question entirely. Spa Beerland rents the whole beer-spa suite to one booking party, while Hotel Salvator and several hotel spas sell private sauna or wellness sessions. If mixed-gender nudity, gender separation, or privacy is important to you, choose the facility by this rule before you compare the number of saunas.
Treatments, Robes, Draping, and Privacy
Treatment clothing depends on the procedure. For a massage, mineral bath, wrap, or body treatment, staff may ask you to undress to the level needed for the procedure and then drape you with a sheet or towel. Grandhotel Pupp’s spa etiquette, for example, says that most procedures require removal of clothing and that the therapist covers the guest with a sheet while exposing only the area being treated. It also says a swimsuit may be worn for a treatment, although oils and creams can reach the fabric.
Do not assume that a medical treatment is a communal nude experience. Baths, wraps, physiotherapy, massages, and many balneological procedures take place in individual treatment rooms or cabins with staff instructions for clothing and draping.
Quiet, Phones, Hygiene, and Shared Space
Shower before pools and heat rooms, sit or lie on the required towel or sheet, and leave enough room for other guests. Keep voices low in relaxation areas. Saunia prohibits phone calls and audiovisual recording inside Sauna World, and Grandhotel Pupp asks guests to leave mobile phones and valuables out of the spa.
Alcohol and intense heat are a poor combination, and medical treatment adds its own restrictions. Follow the facility’s health rules, tell treatment staff about relevant conditions or medications when asked, and do not treat a sauna, mineral bath, or drinking cure as something that must be completed if you feel unwell.
Public Spas in Karlovy Vary
Karlovy Vary’s official public-baths directory names four facilities, but “public” does not mean the same experience at each one. Saunia is a modern thermal-pool and sauna resort, while Elisabeth is a historic municipal bathhouse with treatments and swimming. Windsor continues the medical role of the former Spa III, and Carlsbad Plaza combines an outpatient medical program with separate day access to a large hotel wellness center.
Elisabeth Spa (Alžbětiny lázně / Spa V)
Best for: a traditional public bathhouse where you can book treatments without staying in a spa hotel.
Elisabeth Spa is the clearest modern example of a stand-alone Karlovy Vary spa: it does not provide accommodation, but it has treatment departments, rehabilitation, wellness services, and a public swimming complex. Its treatment menu runs from classic mineral-water and rehabilitation procedures to massages, beauty, and relaxation care.
The pool complex is useful if you want swimming as well as treatment. The 25-meter pool uses chlorinated freshwater, with a separate relaxation pool, whirlpool, sauna, and steam areas. That makes Elisabeth a good example of why “spa” and “thermal mineral-water pool” are not interchangeable.
You can book selected procedures individually or arrange a medical consultation. Check the current treatment schedule and pool access directly before visiting because treatment departments and swimming facilities may operate on different timetables.
Clothing & gender: Pool: clean purpose-made swimwear is mandatory; ordinary underwear, non-swimming shorts, burkinis, swimming hijabs, and full-body suits are prohibited under the current rules. Sauna and steam: remove swimwear, shower, and enter nude and barefoot. Gender: sauna and steam areas are separated for men and women; the post-sauna rest room is shared.
Windsor Medical Complex & SPA Hotel (Former Spa III)
Best for: traditional medical-spa treatment in one of Karlovy Vary’s historic public-bath buildings.
Windsor occupies the former Spa III building beside Mill Colonnade. The building dates from 1866 and was created as a major balneological institution; today the complex combines hotel accommodation with medical spa care, a swimming pool, and sauna facilities.
Windsor belongs to the city’s current public-baths list, but it is more treatment-led than a casual municipal swimming bath. If you are not staying at the hotel, confirm which treatments, pool sessions, or day services are available to outside visitors before you go.
Clothing & gender: Pool/wet areas: the current public information does not state a specific swimsuit rule. Sauna/heat rooms: no zone-by-zone textile rule is published. Gender: no regular mixed-versus-separated schedule is published. Ask Windsor directly before a day visit if any of these rules affect your choice.
Saunia Thermal Resort
Best for: a few hours of thermal bathing, saunas, cooling, and relaxation without entering a medical treatment program.
Saunia Thermal Resort, beside Hotel Thermal, is the easiest public choice when your priority is pools and sauna rather than physician-led care. Its signature feature is the outdoor pool filled with 100% Karlovy Vary spring water at about 38°C. A larger pool at about 32°C is intended for swimming and relaxation and includes water attractions.
The sauna world has a broad heat-and-cold circuit with Finnish, herbal, salt, steam, tropical, and other themed rooms, plus cooling pools, showers, a bucket, and an ice well. This is wellness rather than a traditional Karlovy Vary cure: you can experience the thermal water and sauna culture without a medical examination or treatment plan.
Clothing & gender: Pools: use clothing suitable for bathing; shower thoroughly without a swimsuit before entering the water. Sauna World: entry is only without a swimsuit; use the supplied sheet or towel as required. Gender: Saunia does not publish a recurring male- or female-only Sauna World period; follow signs for sex-specific changing or support spaces.
Carlsbad Plaza: Ambulatory Treatment and Wellnessland
Best for: either a serious outpatient medical program without hotel accommodation or a luxury public wellness day.
Carlsbad Plaza has two public-facing options that should not be confused. Its ambulatory treatment program is a medical course for people who stay elsewhere. The program begins with a medical examination and laboratory assessment, runs for at least seven days, and can include up to 18 prescribed treatments along with access to the hotel wellness facilities.
For a leisure visit, ALT KARLSBAD Wellnessland also sells day access. The large complex has multiple pools and heat rooms, including a swimming pool, whirlpool, massage pool, mineral-water pool, Japanese acupressure pool, and a sauna village with Finnish, Russian, infrared, salt, herbal, steam, and lower-temperature relaxation rooms.
Choose the outpatient program if you want medical balneotherapy without moving into a spa hotel. Choose Wellnessland if your goal is pools, saunas, heat, cold, and relaxation for a day.
Clothing & gender: Pools: the current public pages do not state a specific swimsuit rule. Saunas: sheets, sauna hats, towels, and slippers are supplied, but a complete textile rule is not published for all seven sauna types. Gender: no recurring male/female separation schedule is published. Confirm the current Wellnessland rules before a day visit if sauna nudity or mixed use matters to you.
Hotel Spas and Medical Spa Houses in Karlovy Vary
Karlovy Vary has far more hotel spas than public bathhouses. The inventory below focuses on properties with a distinct in-house balneology department, physician-led treatment program, or substantial pool-and-sauna center.
NOTE: Hotel websites often describe pools and saunas without publishing the dress code or whether a heat room is mixed, separated, or scheduled for one gender. Where a current operator rule is explicit, it is stated below. Where an entry says “not published,” do not fill the gap with general Czech sauna custom—ask the hotel before booking if the rule matters to you.
Full Medical Spa Resorts
Hotel Imperial
Hotel Imperial is one of the strongest choices for a traditional medical-spa stay with modern diagnostics and rehabilitation. Vřídlo mineral water is piped into the hotel for prescribed drinking cures, baths, and irrigations. Treatment packages can include medical examinations, laboratory testing, physiotherapy, mineral baths, massage, diet advice, and other procedures.
The wellness side includes an indoor pool, whirlpool, and sauna, while the medical program extends beyond classic balneology to equipment such as a hyperbaric chamber. Choose Imperial when you want a large, self-contained medical resort rather than a simple hotel wellness floor.
Clothing & gender: Pool/whirlpool: not published. Sauna/steam: not published. Gender: no current separation schedule is published. Confirm these rules with the hotel if they affect your choice.
Spa Resort Sanssouci
Spa Resort Sanssouci combines several connected buildings with a large medical-spa operation. Treatment begins with a medical examination and can use Karlovy Vary thermal mineral water, natural carbon dioxide, and peloids alongside physiotherapy and modern procedures.
The resort also has a swimming pool, whirlpool, and sauna, so it works for couples where one person wants a structured medical program and the other wants more conventional wellness time. The connected buildings reduce the need to go outside between rooms, dining, and treatment departments.
